Questions for real requests
Short and long answers, email, phone, number, date, one or several choices, drop-downs, 1–5 ratings and headings between sections.
Forms
Turn a printed code into an enquiry, registration, feedback or quote request form. Responses land in your inbox.
Quick answer
A form QR code opens a short form hosted by QRflick. People answer on their phone without an account; you read the responses in an inbox, get an email for each one and export them to CSV. Every form asks for consent and says how long the answers are kept.
Short and long answers, email, phone, number, date, one or several choices, drop-downs, 1–5 ratings and headings between sections.
People tick a consent box that names who collects the answers and how long they are kept. On paid plans the form also links to your privacy policy.
Choose to delete responses after 30, 90, 180 or 365 days, or keep them until you delete them. The Free plan has a shorter maximum.
Search responses, delete the ones you no longer need and export them to CSV. Editing or reordering questions keeps earlier answers.
Get a notice when someone sends the form, and switch it off in your notification settings.
Hidden spam traps and rate limits stop most automated sending. Forms that ask for passwords, card or bank details, ID numbers or wallet keys are refused.
How it works
Add the questions you need, from a name and email to a date, a choice or a 1–5 rating. Choose who collects the answers and how long you keep them.
Put the QR on a flyer, table card, van or quote. People open the form on their phone and answer without an account.
Each response arrives in the form’s inbox and, if you like, by email. Search, export to CSV or delete it when you no longer need it.
Use cases
Ask for the job, the address area and the best time to call, so you can reply with a useful first answer.

No IP address is stored with a response, only the answers, the consent text shown, the time and an approximate country. Our privacy policy and acceptable use policy explain this in full.
The Free plan includes one form. Paid plans add more forms, more questions and responses per month, a privacy-policy link and a redirect to your own page after sending. Current limits are on the pricing page.
